(b) Two (2) copies of certificates/Testimonials
(c) Birth certificate or statutory declaration of age
(d) Names of three referees who should be asked by the candidates to forward referees report direct to the above addressee
(e) Candidates must attach evidence of their Local Government Areas, indicating state of origin and signed by Local Government Chairman/Secretary
